Initial Assessment and Comprehensive Case Review are assessment levels. Remote, In-Person, and Hybrid Assessment are formats. FHI reviews the intake and confirms both the appropriate level and format before booking.
Hybrid cases are assigned to the appropriate Initial Assessment or Comprehensive Case Review fee after intake review.

Initial Assessment — $400
For patients seeking a structured first assessment, including those with early hair change, limited prior records, or a relatively straightforward history.
Includes routine review of the submitted intake, photographs, and relevant available records.
Comprehensive Case Review — $600
For cases requiring substantial pre-visit review of a longer or more complex history, including multiple prior assessments, conflicting findings, extensive records, or several treatment trials.
The appropriate assessment level is confirmed before booking.
FHI reviews the submitted intake before confirming whether an Initial Assessment or Comprehensive Case Review is the appropriate route. The applicable fee is confirmed before booking. There is no charge for the 15-minute fit call.
Payment and coverage
Private services are patient-paid. A receipt is provided. Patients may be eligible to claim benefits directly from a health spending account or private insurance plan. Applicable sales taxes are added for patients in Canada. Payment methods and the cancellation or rescheduling policy are confirmed at booking.
