Begin the FHI intake
Start here for a first assessment, a review after diagnosis, treatment-response interpretation, long-term monitoring, or assessment around a procedure.
How intake begins
You will create a secure patient-portal account, complete the intake, and submit relevant photographs and records. FHI reviews the material, identifies important gaps, and confirms the assessment route and booking next step.
Before you submit
- Prepare a concise timeline of onset, change, relapse, recovery, and major health or life events.
- Describe the areas involved and whether the main concern is shedding, thinning, patches, breakage, scalp symptoms, relapse, or treatment response.
- List current and prior treatments with dates, consistency, benefit, adverse effects, and reasons for stopping.
- Prepare recent and earlier scalp photographs where available.
- Gather relevant laboratory results, prior consultation notes, trichoscopy, biopsy or pathology reports where available.
- Identify the decision or question you are trying to address.
